  • NDIS participant number and plan details
  • Health, disability, and support needs information
  • Emergency contact information
  • Financial information for billing and claiming purposes
  • Communication records including emails and notes from support sessions

3. How We Use Your Information

Your information is used to:

  • Deliver and coordinate your NDIS supports
  • Communicate with you, your family, carers, and support coordinator
  • Submit claims to the NDIS and process invoices
  • Meet our reporting and audit obligations as a registered NDIS provider
  • Improve our services and staff training
  • Comply with legal and regulatory requirements

4. Disclosure of Your Information

We will not disclose your personal information to third parties without your consent, except where required by law or necessary to deliver your supports. This may include the NDIS Quality and Safeguards Commission, the NDIA, allied health professionals, or emergency services where your safety is at risk.

5. Storage and Security

Personal information is stored securely using industry-standard protections. Digital records are password protected and access-restricted. Physical documents are kept in locked storage. We retain records for a minimum of 7 years as required under NDIS obligations, or longer if required by law.

You have the right to:

  • Access the personal information we hold about you
  • Request corrections to inaccurate information
  • Withdraw consent for non-essential uses of your data
  • Lodge a complaint about how we handle your information
